About the Role

Join Intermountain Health as a Clinical Pharmacist specializing in anticoagulation therapy. This position ensures safe, effective, and cost-efficient pharmaceutical care focused on managing complex medication regimens for patients requiring anticoagulation. You’ll work closely with a multidisciplinary healthcare team to optimize patient outcomes while providing education and professional support.

Key Responsibilities

  • Review and evaluate medication orders for allergies, interactions, duplication, and therapy appropriateness.
  • Provide evidence-based interventions to optimize medication use and safety.
  • Oversee timely preparation, delivery, and secure storage of medications.
  • Counsel patients, promoting medication adherence and education.
  • Comply with all state and federal pharmacy regulations and maintain accurate documentation.
  • Supervise and support interns, technicians, and other pharmacy personnel.
  • Collaborate with healthcare providers to offer clinical consultation and clarify medication-related queries.
  • Identify opportunities to improve processes and patient satisfaction.
  • Participate in pharmacy leadership initiatives and quality improvement efforts.

Required Qualifications

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
  • Licensed to practice pharmacy in Montana or Colorado (or able to obtain license by start date).
  • Current knowledge of federal and state pharmacy laws and regulations.
  • Strong communication, project management, and people skills.
  • Immunization certification if applicable.

Preferred Experience

  • Experience managing complex anticoagulation therapy.
  • Previous work in an integrated healthcare delivery system.
  • Board certification and completion of a health system residency are a plus

Physical Requirements

  • Ability to read and assess information, labels, and equipment.
  • Effective verbal communication with patients, providers, and colleagues.
  • Manual dexterity for handling delicate pharmacy equipment and technology use.
